Hi. Male 54 y/o, on haemodilysis for about 1 year and a half, 3 times per week, 4 hrs. PTH 957 pg/mL, Ca: 8.1 mg/dL , P: 6.7, Mg: 2.3. What treatment options don you sugest for hyperpara? Thank you

In this case, cinacalcet would be a reasonable option especially in light of the hyperphosphatemia. However, I would first ensure the hypocalcemia has been corrected and, throughout treatment, calcium needs to be monitored. Activated vitamin D is also a consideration but care would have to be taken to control phosphate with your binder of choice.
