A 65 year-old man presented to hospital with crushing retrosternal chest pain and was diagnosed with a transmural myocardial infarction. His background included a history of diabetes and osteoarthritis for which he was on metformin and ibuprofen respectively. He was treated with primary angioplasty but within 48 hours, developed acute renal failure. The following was noted on microscopy:
