I ask about the best choice for treatment of resistant hypertension in polycystic kidney disease.

Hello,

It has been shown that cysts induce focal ischemia and stimulation of the renin-angiotensin system. Therefore, ACE inhibitors and ARBs likely work well. There has been some concern expressed with respect to calcium channel blockers and their effect on cyst growth. Some authorities choose other agents before resorting to CCBs. However, blood pressure control is obviously paramount and so CCBs would still be a viable option albeit perhaps lower down in the algorithm.
