I have a pt who has CRF and is taking 200 mcg of Synthroid who now has a TSH level of 19.44.  What is the appropriate method of treatment for this pt.??

There is no difference in the treatment of hypothyroidism in patients with CKD and in those without CKD. If TSH remains elevated and sufficient time has passed between the introduction or increase in dose, then one might need to use more thyroid replacement. These recommendations are generalized only and may not pertain to your individual patient.
