Creatinine high

My creatinine ratio is 4.2....earlier it was 3.7...it is in this condition for the past 1 year...previously it was 2.7....also urine protein ratio is high...I am taking medication for blood pressure lercadip 10 mg twice...crestor 10 mg for cholesterol and adenuric 80 for uric acid...
Please advise how to reduce this
