I personally would not use sublingual products aimed at rapid BP lowering. We have learned from the experience with sublingual nifedipine, for example, that the speed with which blood pressure is lowered might be dangerous. Furthermore, most patients with intradialytic hypertension might rapidly correct once volume removal begins. If they do not, these patients should have their target weight lowered until euvolemia and then placed on long-acting drugs rather than sublingual, short acting ones.
