What might be the most important for family doctors to know?
Probably the most significant change relates to the target blood pressure for patients with NON-DIABETIC kidney disease. Previously both diabetic and non-diabetic patients had a suggested BP target of <130/80. As of the 2012 guidelines, CHEP now recommends patients with NON-DIABETIC kidney disease achieve a target of <140/90. This change in target comes as a result of recent studies and reanalysis of older studies showing no benefit of BP <130/80 in patients with non-diabetic kidney disease, and in fact, some suggestion of harm with the the lower target. The one caveat is that patients with proteinuria might indeed benefit from targeting <130/80, even in the absence of diabetes.
