This represents a severe loss of kidney function. In and of itself however it does not dictate whether dialysis is required urgently. You must discuss this with a qualified physician in person (as opposed to online) as more information is required to help decide whether dialysis is required now. This would include measurement of serum potassium, bicarbonate as well as a detailed assessment of symptoms and a physical examination.
