good presentation but I still believe that AVF should be the first choice in all patints unless cotraindicated or deemed impossible.Putting a catheter in an elderly with multiple comorbidities will also add more mordidities and create a chronic inflammatory state with its attending risk for malnutrition, sepsis and resistance to ESAs etc..all of which can worsen the prognosis in such pattients.
