I am a practicing nephrologist in US starting to think that I should be adding spironolactone - even low dose - to an ARB or an ACEI for most nephrotic syndromes, and for diabetic nephropathy - if the potassium can still be controlled then what is the harm? any thoughts ? thank you

thank you appreciated !
I agree completely. It will be very interesting to see the results of the clinical trial with finerenone added to ACEi or ARB, seen here:
