I have been diagnosed with primary hypertension. all my investigations are normal: Urine protein, Urine creatinine, urine noepinephrine and dopamine, Kidney Function Test, Liver Function Test and Thyroid Tests. My kidney scans are normal and eGFR is normal as well. electrolytes and fasting blood sugar are normal too.
But my renin is very high 291.10 ng/L (normal is 2.6-27.7). My aldosterone is normal so as with my aldosterone and renin ratio is normal. I was taking candesartan 32 mg a day for 1.5 years already but now shifted to 2 weeks exforge lowest dose. my worry is about my renin value why it is so high and what is the indication of it? what medication can be given to me in this case. thanks you very much for the answer.

High-renin hypertension is very common and for the most part, not indicative of anything different than if renin was not elevated. However, in rare cases the renin elevation can be secondary to renal artery stenosis or even more rarely due to a tumor that secretes renin. To investigate the more rare causes, you could undergo a renal MRI with MRA to look for a small renal mass and to rule out renal artery stenosis. But I must stress, that in my experience of investigating renin elevation, I have very seldomly found a cause.

I had the same situation one year ago. My doctor said ARBs and ACE inhibitor all increase plasma renin. Candesartan belongs to a class of drugs called angiotensin receptor blockers (ARBs).
