What is your suggested work-up for incidental hematuria on an insurance examination in a middle-aged male?

Personally, I would work this up especially if persistent. This is a very common reason for referral to my office and in most cases is benign (even idiopathic).

Thanks for the reply - to clarify, would you suggest any investigations at all in a fit 40yo male with a single instance of an asymptomatic finding of 6RBC/HPF?

The answer to this question would depend on the patient's age.
In general though, patients should undergo urine culture to exclude infection, ultrasound to assess for urinary tract and renal structural abnormalities, renal function testing, urine cytology and possibly cystoscopy, particularly in older patients who might be at increased risk of bladder neoplasms. Of course, each patient needs to be examined as well to assess for hypertension as well of signs if systemic disease.
