Why is Aspirin and Statins should be given only at night?

Hello,

There is not great evidence for either recommendation.

1. Regarding statins, there is a theory that cholesterol synthesis is largest when one is not eating, hence the rationale to ensure highest levels of statins when one is asleep (and fasting). Two papers supporting this are:

  • TM Lund et al. Effect of morning versus evening intake of simvastatin on the serum cholesterol level in patients with coronary heart disease. Am J Cardiol 2002 90: 784-786.
  • A Wallace et al. Taking simvastatin in the morning compared with in the evening: randomised controlled trial. BMJ 2003 327: 788.


2. Regarding ASA, there is ample evidence that most MIs occur in the AM. However, since most platelets are inhibited once ASA has reached steady state (and possibly before), there is probably not much need to give ASA at night. Though, there is little downside as well.
